For those who are making plans for a loved-one who may be nearing death. The primary difference in the forms is the Authorization for Cremation. The Pre-Need Authorization assumes that the individual is still living and will authorize their own cremation or it will be authorized by the person with Healthcare Power of Attorney. We will need a copy of the Power of Attorney for our files.
For those who are making arrangements for someone who has died. These forms include the South Carolina Death Certificate Worksheet as well as the At-Need Authorization for Cremation to be signed by all legal next of kin.
